Document Required:-

Establishment Details:
1. Name of the establishment, Address, Incorporation Date, PAN and Type of establishment.

2. If the establishment is a factory then the details to be provided are the Factory License Number, Date of License and Place of issue of License.

Applicant Details:

1. PAN Card of the Proprietor/Partner/Director.
2. Aadhar Card of Proprietor/Partner/Director.
3. Proof of address such as the Electricity Bill or Telephone Bill of the Registered Office (not older than 2 months).
4. Shop and establishment Certificate/GST Certificate/ any License issued by the government for the establishment.
5. Cancelled Cheque or Bank Statement of Entity.
6. Hired/Rented/Leased Agreement of the registered office.
